Qualifications to Enroll

Chicago R.E.A.C.H. serves seniors who are:

  • At least 55 years of age
  • Live in the Chicago R.E.A.C.H. service area
  • Assessed by the Chicago R.E.A.C.H. interdisciplinary team
  • Screened by the State of Illinois and found qualified for Chicago R.E.A.C.H. services

Funding Source

Illinois Department of Healthcare and Family Services pays Chicago R.E.A.C.H. a capitated monthly rate. Medicare services are billed as fee for service.
