The Chicago R.E.A.C.H. Program
Chicago R.E.A.C.H. (Reaching Elderly Across Chicago’s Horizon) is a not-for-profit organization serving the frail seniors in Chicagoland since 1995.
The R.E.A.C.H. program provides a holistic approach based on the individual’s need. R.E.A.C.H. provides a unique financial model in meeting health care needs for the frail elderly. Chicago R.E.A.C.H. provides full medical care, social service, rehabilitation, and recreational activities to keep seniors in their community setting.
Chicago R.E.A.C.H. is a recognized member of the National PACE Association. Programs of All-inclusive Care for the Elderly (PACE) provide and coordinate all needed preventative, primary, acute, and long-term care services so that older individuals can continue living in the community.
The R.E.A.C.H. Team
Chicago R.E.A.C.H.’s interdisciplinary team members are qualified professionals dedicated to serving seniors. They recognize the dignity of each senior’s life and extend compassion in every encounter.
