A Real Alternative to Nursing Home Placement

Chicago R.E.A.C.H. Foundation is an innovative holistic health care program dedicated to enhancing the quality of life for the frail elderly of the Chicagoland community. Chicago R.E.A.C.H. provides comprehensive services designed to maximize the senior’s potential and maximize their ability to remain in the community. Chicago R.E.A.C.H., in collaboration with the National PACE Association (Program of All-inclusive Care for the Elderly), is a comprehensive service delivery and financing model of acute and long-term care.

The goal of the program is to maximize each participant’s autonomy and continued community residence and to provide quality care at a lower cost to Medicare and Medicaid.

Chicago R.E.A.C.H. promotes the wellness and independence of our frail elderly through comprehensive, coordinated services that are provided in partnership with program participants, their families/caregivers and the interdisciplinary team. The program is dedicated to supporting an environment sensitive to the physical, psychological, spiritual, social, cultural and economic needs of both the frail elderly and their families/caregivers.
